What the form must show

  • The owner’s full name and contact details, plus the ABN if it is a business.
  • The installation address, matching the registry claim exactly.
  • The unit details: brand, model, serial number and tank size.
  • The installation type: new, replacement or additional.
  • A clear statement that the owner assigns the right to create STCs to the installer or their agent.
  • The owner’s signature and the date, before the claim is lodged.
  • A declaration the owner understands what they are assigning, including the value passed on to them.

Heat-pump-specific points

The installation type matters most. If the unit replaces a gas or electric resistance heater, the form and photos need to support that.   • Unsigned or undated forms.
  • A form signed after the claim has been created.
  • Different spellings of the address between form and claim.
